Senate Bill 12 Requirements

Texas Senate Bill 12 requires all students to have a consent form on file to receive ANY health and/or counseling-related services, including first aid and illness care while at school as well as consent to participate in district/school organizations or clubs. Please complete and return these forms as soon as possible to your child's teacher or the principal's office. If you have not received these forms, do not have access to the forms, or have any questions, please contact the school office @ (979) 884-3366.
